Completed via the CIBT portal or the State Department website. Proof of Departure:
Complete CIBT's online form and upload a scan of your passport's information page. A passport expert reviews your data for errors.
: Your current passport was issued within the last 15 years (and you were 16 or older at the time).
If you don't meet these, you need a "New Passport" application (Form DS-11), which CIBT also handles.
Must meet strict government requirements (2x2 inches, white background, no glasses). Form DS-82:
You upload a digital photo via the mobile app. CIBT prints it on official photo paper. You also e-sign a power of attorney allowing CIBT to act on your behalf.
